This survey is for parents of students currently (or recently) on an Individualized Education Program (IEP). Your responses will help guide efforts to improve services and results for students. Your response is voluntary.

The Lindsey Nicole Henry (LNH) Scholarship Act authorizes the parent or guardian of a public school student with a disability who is served under the Individuals with Disabilities Education Act (IDEA) to exercise their parental option and request to have an LNH Scholarship awarded for their child to attend a participating private school approved by the State Board of Education.

As the parent(s) of a child who is receiving or may be eligible for special education and related services, you have certain rights according to State and federal laws.
